When we open your patient file, we collect the following personal information: surname, first name, postal address, e-mail address, telephone number, date of birth, gender identity, occupation, reason for consultation, medical history, etc. When you contact us via our online form on our website, we collect the following information: surname, first name, e-mail address, telephone number and any notes you write to us (optional notes). When you book an appointment online via our website, you are redirected to the Jane platform. New patients must provide the following information: surname, first name, postal address, e-mail address, telephone number, date of birth, gender identity, occupation, reason for consultation, medical history, etc. Active clinic patients can log in with their username, e-mail address and password only (no other data is required).
Purpose of collecting personal information
We use the personal information we collect for the following purposes only: to identify you, contact you, bill you, schedule appointments, issue receipts, respond to your requests (in person or by e-mail), manage services, operate the clinic and provide you with safe, effective and appropriate treatment via a referral form.
Consent, collection, use and disclosure
The people who have access to your personal information are the administrative staff and the chiropractors you consult at La Clinique Chiropratique St-Sauveur. We obtain your consent before collecting, using or communicating your personal information to third parties, except in the cases provided for by law. Under no circumstances will the CCSS use or disclose personal information without your consent unless it is authorized or required to do so under applicable legislation.
Retention period
We retain your personal information for a period of seven years following its last use by Clinique Sport Santé Laurentides, except where the law provides for a different retention period.
